The Physical Therapist (PT) will be responsible for providing skilled physical therapy services consistent with professional and regulatory standards and policies. Participates in the development of individualized programs necessary to achieve patient care objectives.
Perform comprehensive patient assessments to determine functional deficit and potential and establish a therapeutic plan of care to include goals
Document, within appropriate time frames, observations, patient's response to treatment, treatment programs, degree of progress and notable changes in patient condition
Complete all documentation required and in accordance with state and federal regulations and company policy
Communicate and coordinate patient care with other members of the patient's health care team
Supervise physical therapy assistants in accordance with regulations and company policy
Provide patient and family teaching related to the patient's physical therapy program and its maintenance
Assist the patient and/or family to procure needed equipment or supplies
Provide physical therapy in accordance with physician's orders and accepted professional practices
Utilize appropriate hand washing, standard precautions and personal protective equipment
Maintain skills and education by attending continuing education and in-service programs
Attend employee meetings and patient conferences
Graduate of an accredited Physical Therapy program
Current license as a PT in the state of Massachusetts
One year of experience in home care, or working with elderly patients preferred
Must possess a valid MA driver s license and reliable transportation
Demonstrated proficiency with the administration of objective standardized tests and measures and compliance with regulatory standards
