As an Anthem subsidiary, we benefit from the scale andresources one of America's largest managed healthcare organizations. **Position Overview** The Care Manager RN is alicensed clinician who works closely as part of a multidisciplinary team toidentify and execute the best plan of care for chronically ill patients. TheCare Manager RN manages a census of patients through both responsive andproactive outreach, identifying acute issues and educating on the progressionof chronic illnesses. The Care Manager RN builds rapport telephonically withpatients while walkingthrough the end stages of the patient's disease process and helping themidentify their Goals of Care. **Responsibilities** - Work closely with themultidisciplinary team of Advance Practice Providers, - Social Workers, andNursing Support Specialists to manage a census of patients by helping toidentify and carry out a plan of care - Conduct telephonicassessments of acute and chronic conditions to include acute symptommanagement, medication reconciliation and Goals of Care Conversations under thedirection of the Advance Practice Providers and Physicians - Conduct proactive, telephonicoutreach to patients with chronic, life limiting illnesses to help with Diseaseeducation, medication reconciliation and Goals of Care Conversations - Ensure collaboration withother providers including external physicians and specialists, hospice agenciesand Aspire's multidisciplinary team - Serve as the liaison for thehospice agencies to help ensure smooth and timely hospice transitions - Participate inInterdisciplinary Team Meetings - Perform miscellaneousjob-related duties as assigned - Regular attendance is anessential function of the job **Knowledge/Skills/Abilities** - Minimum of 3years' clinical experience required, preferably in a centralized caremanagement team environment - RN Certificationwith current encumbered license - Hospice andPalliative Nursing Certified a plus - Experienceworking in a team environment that includes a significant amount ofcollaboration and innovation preferred. - Prior healthcareexperience serving chronically ill patients preferred - Hospice orPalliative Care experience a plus - Telephonic casemanagement or care coordination experience preferred - Strongcustomer-service orientation - Compassionateand professional demeanor required - Exceptionalorganizational skills and ability to multi-task - Self-starterwith an entrepreneurial spirit and demonstrated problem solving skills - Strong verbaland written communication skills - Strong Word,Excel, and database management skills - Extremely strongwork ethic **Physical Requirements** The physical demandsdescribed here are representative of those that must be met by an employee tosuccessfully perform the essential functions of this job. **Travel** Minimal to no travel isexpected for this position.
