The Registered Nurse is responsible for providing nursing care, in accordance with agency and agency policy and procedure and the Nurse Practice Act, to an individual in his/her home or in a facility. + Carries out the agency's mission, philosophy, goals and objectives within guidelines of agency policy and position + + Interprets and implements the Agency's philosophy to staff and members of the + + Provides direct nursing care including administration of prescribed medications and + + Initiates nursing measures to prevent complications or to minimize the disabling effects of disease or + + Extends physical therapy services by providing rehabilitation aspects of nursing care and/or under the guidance of a physical therapist, gives therapeutic + + Provides emergency + + Observes, evaluates and reports the patient's physical and emotional condition, reactions to drugs and treatments to the Clinical Supervisor and/or physician as + + Obtains laboratory + + Maintains accurate and timely reports and + + Contributes to the development of the plan of + + Contributes to the care of the patient which may include meal preparation, personal laundry, maintenance of a clean and safe patient environment, securing supplies, + equipment, and/or medications. + Obtains physicians orders including preparing and securing proper documentation in coordination with the Clinical Supervisor. + + Participates in patient/family teaching. + + Participates in case conferences as indicated. Collaborates with other team members. + + At the discretion of the Clinical Supervisor may conduct initial assessment visit, develop plan of care, and function as the primary nurse for the patient under the direction of the Clinical Supervisor. + + Supervises the Aides when functioning as the primary nurse. + + Performs specialized care for which education/experience is documented. + + Maintains patient record in accordance with agency policy and professional standards. + + May take call during non-business hours. + + Carries out all other related duties as assigned. + + Participates in Performance Improvement activities as required. + + Participates in evaluating overall position performance, goal setting and achievement, and performance improvement plan. + + + Current licensure to practice as a Registered Professional Nurse in New York State + Minimum of one (1) full-time year of supervised clinical nursing experience in a hospital OR + Current supervised nursing experience providing skilled nursing care in a hospital, certified or long term home health agency or skilled nursing facility OR Documented relevant clinical experience/training for specialty assignments.
