Description Join our growing, dynamic, accountable care department as an MSW Care Coordinator. We are committed to providing our patients, physicians, advanced practitioners and payers with population health solutions that promote higher quality healthcare at a lower cost. As an MSW Care Coordinator you will be involved in transitional care management and high risk patient management. You will work closely with our medical director, physicians, RN Care coordinators, support staff, and community partners in a team environment. Our organization encourages personal and professional development. Hours are 8 to 5 with no call, night or weekend coverage. Arizona Community Physicians (ACP) is a predominantly primary care, physician-owned, independent medical group composed of 131 physicians, 37 advanced practitioners, and nearly 800 employees. We service Tucson, Green Valley and Oro Valley with 53 clinical offices, two imaging centers, a testing center and a laboratory. Responsibilities
*Provide transitional care management services to our patients recently discharged from hospitals and SNFs *Provide case management services to our highest risk patients *Work closely with our medical director and collaborate with personnel from hospitals, SNFs, home health, and community agencies to develop innovative and efficient solutions to vexing care coordination problems Skills/ Requirements MSW degree and active license are required as well as prior experience in case management, care coordination, or home health. This position requires skills in communication, problem solving, team work, patient advocacy, persistence, persuasion and compassion. The position involves a lot of computer and phone time interacting with patients and providers, but also face to face meetings with payers, patients and community resources.
