PRIMARY FUNCTION:

The Community Health Worker (CHW) will work in coordination with a multidisciplinary team to execute on care plan objectives

PRINCIPAL DUTIES AND RESPONSIBILITIES:

  • Works with individuals assisting navigation of social service agencies, community sites and partners to meet social needs
  • Provides assistance with filling out complex forms
  • Uses innovative and culturally sensitive strategies to engage individuals and families
  • Advocated on behalf of vulnerable community members
  • Works closely with social work to create and execute on a patient centric plan
  • Works closely with multidisciplinary team to improve the psychiatric, medical and social health of individuals
  • Uses multiple modes of patient communication and visits such as office, phone, telehealth and in home visits
  • Timely documentation in an accurate and concise manner, using proper technology, as established in department and CCHS guidelines
  • Works collaboratively within the patient care team, other patient provider teams, with community, state and federal agencies
  • Performs performance improvement activities for the program and department , actively seeking to improve care delivery effectiveness and efficiency, outcome measures and patient satisfaction
  • Demonstrates compliance with Christiana Care Health System organizational and departmental policies, procedures and mandatory educational requirements including but not limited to PEAT, safety, infection control, attendance and dress code
  • Demonstrates compliance with federal regulations
  • Performs assigned work safely, adhering to established departmental safety rules and practices; reports to supervisor, in a timely manner, any unsafe activities, conditions, hazards, or safety violations that may cause injury to oneself, other employees, patients and visitors.
  • Assumes responsibility for professional growth, shares knowledge with co-workers and health care team members
  • Performs other related duties as required.
  • EDUCATION AND EXPERIENCE REQUIREMENTS:

  • High School diploma or equivalent.
  • Two years' experience working in a healthcare setting.
  • An equivalent combination of education and experience may be substituted.
