Christiana Care Home Healthcare (VNA) is seeking a Casual LPN to work a minimum of 8 hours or more each week between the hours of 8am-4:30pm within Kent County, Delaware. Candidates desiring to work additional hours may do so if they are willing to serve patients in the Middletown, Odessa, Townsend area. The successful candidate will work holiday and weekend rotations.
Duties and Responsibilities:
Give skilled nursing care and prescribed treatments to geriatric patients in the community.
Provide care including but not limited to: Cardiac Care, Foley Catheters, Pulmonary Care, Ostomy Care, etc.
Educate patients and their family members on care, disease process, and nutrition.
Implement care plans, which are goal oriented and patient centered and consistent with available resources.
Support discharge of patients.
Qualified applicants must possess a current DE LPN license, or compact state license
One year of LPN experience required
Previous Home Healthcare and Med-Surgical experience preferred
