The Care Management Support Professional 2 contributes to administration of care management. Provides non-clinical support to the assessment and evaluation of members' needs and requirements to achieve and/or maintain an optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members. The Care Management Support Professional 2 work assignments are varied and frequently require interpretation and independent determination of the appropriate courses of action.
This role requires working Tuesday, Wednesday, and Thursday for a total of 24 hours per week during regular business hours. Occasional weekends are required.
This role requires educating and training HHA's on customer service, brand building, range of motion exercises, hand washing, etc. Administration of PPD and drug testing and orientation for newly onboarded HHAs. The role will require occasional local travel in the field accompanying HHA's to patient visits.
The Care Management Support Professional 2 Understands department, segment, and organizational strategy and operating objectives, including their linkages to related areas. Makes decisions regarding own work methods, occasionally in ambiguous situations, and requires minimal direction and receives guidance where needed. Follows established guidelines/procedures.
* Bachelor's degree in Nursing
* Less than 5 years of Microsoft Office technical experience with Word, Excel, Outlook
* Decision making skills regarding own work assignments and priorities
* Previous experience training HHA's or CNA's.
* Previous experience working with patients in the field
* Master's Degree
* Demonstrated experience with Microsoft Access
