Job Title: Assistant Nursing Care Coordinator

Roles & Responsibilities:

Patient Care

* Acts as a clinical leader to support the dailyfunctioning of the unit on a shift basis.
* Delivers patient care by incorporating the tenets ofthe professional practice model of Relationship Centered Care and through theapplication of the nursing process based upon theory, research, evidence-basedpractice, and approved organizational nursing standards.
* Assesses and evaluates patient care needs andapplies critical thinking skills in patient care management.
* Integrates relevant assessment and interventionskills for the delivery of nursing care.
* Collaborates with the interdisciplinary healthcareteam in the development, implementation, and evaluation of the plan of care.
* Coordinates and prioritizes all aspects of patientcare including teaching, rounding, coaching, and planning \"after hospital care\"and delegating to others as appropriate.
* Educates patients and caregivers, while anticipatingneeds and readiness to learn, about their plan of care, transition of care,promotion of health, and prevention of disease.
* Communicates effectively and professionally withpatient, family and all members of the Interdisciplinary Patient Care Team.
* Manages assignments within the Care Delivery Modelof Modified Primary Nursing and demonstrates allocation of material resourceseffectively.

Patient Experience

* Role models the tenets of Mount Sinai RelationshipCentered Care (MSHRCC) through facilitation of the following actions including,but not limited to, Admission Welcome, Bedside shift Report, HELP, AM/PM Care,5 Minute Sit Down, Purposeful Hourly Rounding, Medication Review, DischargeWrap Up, and MD/RN Unit Collaboration.
* Considers the patient's values, preferences,cultural diversity, expressed needs and knowledge in all aspects of care.
* Establishes and maintains a therapeutic relationshipwith the patient and family.
* Uses evidence-based practices to increaseunderstanding of patients' perceptions of care.
* Initiates service recovery for patients and familymembers who have concerns and escalates to Clinical Nurse Manager/AssistantNurse Manager/Nursing Shift Manager or Patient Relations as needed.
* Demonstrates caring, respect, compassion, empathyand active listening through dialogue, body language and actions.
* Supports staff use of evidence-based practices toincrease understanding of patients' perceptions of care.
* Conducts Purposeful Hourly Rounding (PHR) onassigned patients addressing the 4 Ps.

Quality and Safety

* Practices safe and efficient patient-centered care,identifying and escalating barriers and opportunities for improvement.
* Demonstrates clinical skills and knowledge crucialto quality and safety in the patient population served.
* Implements process improvement strategies based oninstitutional, evidence-based ensure and procedures.
* Engages in formal and informal peer andinstitutional review processes.
* Collaborates with interdisciplinary teams to createand implement quality improvement projects, evidence-based practice activitiesand nursing research studies.
* Contributes to quality and safety practices andrequired compliance measures.
* Implements principles of high reliability to identifyand avoid high risk behaviors to provide a safe environment for patients.
* Escalates potential safety hazards or gaps from bestpractice following institutional chain of command protocol.
* Utilizes institutional information technology bydocumenting nursing practice to support quality and performance improvementinitiatives.


* Coordinates nursing care on a shift basis within aspecific unit using the Care Delivery Model of Modified Primary Nursing.
* Assigns staff for the oncoming shift and recommendsneed for additional staff as necessary.
* Coordinates with Clinical Nurse Manager/AssistantNurse Manager/Nursing Shift Manager to ensure that supplies are appropriatelyutilized and inventory is sufficient for patient care.
* Recommends the progression of new employees withintheir unit based orientation.
* Provides feedback to members of the nursing staffregarding adherence to job obligations, ensure and regulatory requirements inconsultation with the Clinical Nurse Manager/Assistant Nurse Manager/NursingShift Manager or designee.
* Assists in preparing time and staffing schedules inaccordance with unit needs, patient care and contractual obligations.


* Contributes to the environment of care to supportclinical colleagues, patients and their families and members of the healthcareteam.
* Advances clinical competence in nursing practice toprogress from competent to expert
* Projects a professional image to colleagues andcommunicates with styles and methods that demonstrate caring, respect,compassion and empathy.
* Incorporates ethical principles into decision makingfor patient and family.
* Encourages and demonstrates a spirit of scholarship,inquiry, life-long learning and innovation for self and others.
* Acquires knowledge and skills relative to the role,patient population, clinical specialty and local or global health communityneeds.

* Advances as a clinical leader by assuming the chargerole and acting as a patient throughput facilitator and resource person for theunit/clinical service area.
* Articulates the values of research and evidencedbased practice and its application to the environment of care.
* Contributes to the profession of nursing throughparticipation in professional organizations.


* Bachelorof Science with a major in nursing required (BSN); Masters in nursing or healthrelated field preferred.
* Relevantclinical competence in area of nursing practice assigned
* NYSRN License and Registration
* Minimumof 2 years' Med/Surg Nursing experience. Demonstrated leadership skillsand demonstrated skills in unit coordination and patient care programs.
* Must have relevant clinical competence in area ofnursing practice assigned; new graduates must possess current knowledge of thenursing process and its application
* Evidence of ability to maintain interpersonalrelationships, communicate appropriately to others, and work effectively andefficiently to solve problems
* Demonstrates the ability to work in a teamdisplaying effective communication and critical thinking
* Ability to read, write, understand and speak Englishin a clear and concise manner
* Visual, physical, auditory and cognitive ability toassess patient status
* Physical and mental health necessary to care foracutely ill patients with a broad variety of diagnoses and behaviors on a dailybasis
* Ability to walk, stand, lift, stoop, push andrespond quickly in a variety of situations and for an extended period of time
* Manual dexterity to perform a variety of clinicalprocedures relevant to practice area (i.e. venipuncture, urinarycatheterization, cardiopulmonary resuscitation)
* Ability to use basic data entry, retrieval systemsand the electronic medical record documentation system in a proficient manner
