Are you an experienced CNA with a passion of Home Care? Do you enjoy working with patients one on one in the comfort of their own homes? This is the role for you. The Home Care Aide will provide designated care under the direction of the primary service provider in the home in order to achieve identified goals. Function as an extension of skilled nursing services and/or rehabilitation services in the home.
Attends compliance training and adheres to the organization standards of conduct, policies and procedures.
Communicates appropriately with the interdisciplinary home care team in a timely manner, reporting any changes in patient's condition to appropriate professional staff.
Focuses on customer service and continually strives to perform the duties of their job in a manner that will result in optimal patient satisfaction.
Identifies, develops and participates in process improvement opportunities within the home health agency that will enhance the quality of services we provide.
Maintains and completes documentation accurately and promptly while in the patient's home and within the agency and regulatory standards.
Participates in all required agency meetings.
Performs duties and job responsibilities in a fashion which coincides with the service management philosophy of the organization towards patients, visitors, staff, peers, physicians and other departments within the organization.
Proven ability to facilitate change.
Provides safe and direct personal care to patients in the home setting under direct supervision of appropriate professional.
High School Diploma or GED - Required
Must have successfully completed a training and competency evaluation program as specified in 484.80 (Home Health) and/or 418.76 (Hospice) as applicable: Condition of participation: Home Health aide services.
One (1) year experience - Preferred;
Active employment within the past 24 months as a Certified Nurse Aide in good standing.
