Job Title: MAP Intake Coordinator

Roles & Responsibilities:

Responsible for receiving external referrals, making screening calls with potential members and scheduling initial assessments with Enrollment Nurses. Promotes and fosters an ongoing collaborative communication between external stakeholders, referrals and VillageCare. The position is responsible to perform duties to include Medicaid verification and update of enrollment system. The position will play an integral role in assisting and maintaining the Business Development Manager's relationships with all referral sources.

* Screening calls to all external referrals.
* Helps maintain accuracy of data for reporting
* Communicating with referral sources to minimize referral close ratios.
* Working with Business Development Managers to ensure accurate referral data.
* Ensure timely data entry of all referrals
* Analyze Quantity and quality of referrals to report to management
* Ensure timely outbound calls of all incoming referrals
* Perform other job related duties as me be required by the superviso


* Education: This position requires a minimum formal education of Associate Degree or certification equivalent.
* Experience: A minimum of 3 years job related experience
